CÁC BƯỚC LÀM THỦ TỤC HƯỞNG TRỢ CẤP COVID 19 CHO NLĐ CÓ FILE MẪU KÈM THEO
Bước 1: Báo giảm NLĐ nghỉ không lương 1 tháng theo mẫu 600a ( Phương án: KL; Ghi chú: Nghỉ không lương do Covid 19) nộp qua cổng thông tin điện tử. BHXH sẽ duyệt trong vòng 1 tuần.
Bước 2: Nếu DN làm hồ sơ nhận trợ cấp covid là: 1.855.000đ/NLĐ ( nếu NLĐ nghỉ liên tục từ 15-30 ngày) hoặc 3.710.000đ/NLĐ ( nếu NLĐ nghỉ liên tiếp trên 30 ngày) thì sau 3 ngày nộp mẫu 600a thì nộp tiếp mẫu 600g ( đính kèm file PDF mẫu 05- bảng thỏa thuận nghỉ không lương theo QĐ 23) nộp qua cổng thông tin điện tử. BHXH xác nhận vào mẫu 05 sẽ gửi trả lại hồ sơ đã xác nhận cho DN.
Bước 3: Gửi CPN bộ hồ sơ cho phòng LĐ TB&XH của quận nơi DN đăng ký. Phòng LĐ TB&XH duyệt hồ sơ rồi sẽ chuyển khoản vào số TK của NLĐ đã cung cấp trong mẫu 05.
Các hồ sơ bao gồm:
– Mẫu 05 bảng thỏa thuận nghỉ KL có xác nhận của BHXH ký duyệt.
– Bản sao biên bản thỏa thuận nghỉ không lương của NLĐ.
– Bản sao CMND/CCCD của NLĐ có công chứng. Giấy khai sinh của con có công chứng Nếu có ( Nếu NLĐ có con dưới 6 tuổi thì được hưởng trợ cấp thêm 1 triệu).
